A program of care for individuals who need more time and support to complete their restorative care after a hospital stay.
Our hospital-to-home and community transition support provides a safe, comfortable, and home-like environment where individuals can rest, rebuild confidence, and regain independence after a hospital stay. At Tea-cup Cottage, we create calm and welcoming spaces that support wellbeing, personalised care, and a smoother transition back into everyday life.
We’re a registered NDIS and Support at Home Aged Care provider, supporting individuals across South East Queensland and Toowoomba.
Our Services
Tea-cup Cottage’s Transitional Care Program supports individuals through the journey from hospital to home – combining clinical care, a homelike environment and coordinated discharge planning, so recovery happens with confidence.
Clinical and Nursing Care
Hospital-equivalent oversight in a residential setting. Our RN-led team provides round-the-clock nursing care, wound care, medication management and chronic disease monitoring - so recovery continues safely, wherever you are in the journey.
A Place to Recover
A calm, comfortable and homelike environment designed for healing. Residents rest, rebuild confidence and regain independence in spaces that feel welcoming, not clinical.
Allied Health and Rehabilitation
Coordinated access to physiotherapy, occupational therapy, speech pathology, dietetics and social work - integrated into daily routines to support functional recovery and a safe return home.
Fast, Coordinated Discharge Planning
We work directly with hospital teams on pre-discharge planning, assessment and structured transition support — reducing length of stay and giving patients and families confidence in the next step.
